Birch Psychology does participate in out of network reimbursement.
We provide you a super bill receipt with information about the services, diagnosis, etc. that you received.

For therapy, if they ask, the CPT codes used are: 90791- For your 1st session, 90837- Every individual session, 90846- Every parent only session, 90847- Family or couples sessions.
You can request your clinician to send you monthly superbills automatically.

For assessments, if they ask, the CPT codes used are: 90791 (1 unit), then 96130, 96131, 96132, 96133, 96112, 96113, 96136, and 96137. Which are used will depend on the testing battery chosen, but there are several units of each.
Your superbill will be sent to you at the time of your feedback session, not at time of payment.

Login to your online portal: https://www.anthem.com/login/
Claims and Payments- Claim Submission Center_____Submit a Claim- Get Started_____My Medical Plan, Doctor or other medical specialist______Select the patient_____Upload our prepared superbill!

United/ Optum
Sign in to your member account and go to the “Claims & Accounts” tab, then select the “Submit a Claim” tab. There, you’ll be able to select the Medical Claims Submission form to download and print.
After you print and complete the Medical Claims Submission form, mail it with the claim details and receipts to the address on your health plan ID card.

Cigna

Submit a claim online at kp.org/coverageandcosts, sign in to your account, and select Submit a Claim. Then, select Submit Medical Claim Online and upload your itemized bill(s) and receipt(s).

Aetna

Fill out all required fields on the claim form, including provider details, diagnosis codes, and service codes. 

Submit the claim with your superbill from us online through the Aetna portal or mailed to the address on your card.
